Advance Directives

All individuals have the right to make decisions about their healthcare and treatment. NCWBH and contracted providers recognize the individual’s right to develop Medical and Psychiatric Advance Directives outlining their expectations and choices in healthcare delivery in the event they are unable to voice these choices for themselves. Services are provided with due respect for existing Advance Directives when enrollees provide a copy of these directives to their service provider.

More information, including documents in alternative languages, is available on the development of Psychiatric/Mental Health Advance Directives from the Department of Social and Health Services.
